How to be an Appointed Agent

Prospective Agent, thank you for your interest in representing Kingsway Amigo Insurance Company, the following documents are required when submitting an Application for Agency Appointment:

• Brokerage Appointment Application must be completed, including the portion on the second page for estimated volume. (If agency is a new establishment, you are still required to complete the anticipated volume column).
• Current Errors & Omissions Dec Page or Binder.
• Copy of the 2-20 license for the agent that will be appointed in the agency.
• Resume to show experience or business plan.
• If you are interested in a Flood Appointment, please complete the Flood Appointment Form Package and submit with a copy of your agency’s W-9 Form and current E & O Policy.
